- On basis of Availability of bed, Patient can be admitted in the institute round the clock.
- Contact admission counter: 02268190005, 02268190006.
- Patient: Any id proof available for name, age and address verification.
- Doctor's Admission Note: Whenever admission advised by any doctor.
- Mediclaim: Reimbursement facility is available.
- Admission Procedure: An admission application form + Patients rights & responsibilities information (available at registration counter. admission Application Form to be filled by the patient and/or nearest relative, mentioning correct details of patient.
- Deposits: Payment through Cash (upto Rs 1.99L) /Demand Draft/RTGS/NEFT/UPI/Card to be made as per existing schedules for getting admission in the Institute.* Demand Draft to be in name of “Conwest & Manjula S. Badani Jain Hospital Trust Settlement No. XI “.Â
- Bed Allocation: The institute shall allot a bed to a patient as per choice depending upon the availability of beds. In case the bed of choice is not available, arrangements shall be made as per mutual convenience.
- Attandant pass: One day pass and one night pass will be issued to 2 attandants per patient. Please note only one attendant is allowed to stay with the patient at a time.
- Visitor's Pass: Attendentant & visitor (valid in visiting hours only) pass shall be issued per patient in Day Care, Lower median, Higher Median, First and Delux class.
- Visiting hours: 5pm-7pm. A seperate visitor pass will be issued to the visitors during this time by the hospital security. At a time only one visitor will be allowed to visit the patient to avoid crowding in patient areas. Once the 1st visitor submits the visitor pass back to security 2nd visitor will be allowed to visit the patient.
- Once the patient opts for a particular class (even in I.C.C.U.), the institute does not allow transfer to a lower class.
- From Lower to Higher category bed Patient shall be charged as per the rates of the higher class, with retrospective effect from the date of admission in the institute.
- If patient chooses to upgrade the class, patient shall be charged as per the rates of the higher class with retrospective effect from the date of admission.
- Patient vacates the bed occupied prior to transfer. However if the patient’s relative intent to occupy the room it can be arranged on availability of the room, and shall be separately charged at full rates. Prior permission of hospital administrator is mandatory for this arrangement.
- All services are charged and received in Indian Rupees. Payments are accepted in Cash (upto Rs. 1.99L ) /Demand Draft / UPI/RTGS/ NEFT/CARD
- Interim bill memo will be given on daily basis or enquiry has to be made at admission counter for current status of bill.
- Patients are discharged round the clock.
- In case of an unplanned discharge after 6:00 P.M., the final bill shall be prepared after 09:00 A.M. next morning and such discharged is to be covered by a security deposit as found necessary by the management.
- All refunds (Above Rs. 10,000/-) are paid by cheque only, after a period of 1 week from the date of discharge between (Timing 10:00 A.M. to 4:00 P.M.).
- No Refund on Sundays / Hospital Holidays
- All reports of the O.P.D. can be collected from the respective department, within one week from the date of getting the investigations done
- Any I.P.D reports / x-ray films not collected at the time of discharge should be collected from the respective wards / departments within one week from the date of discharge.
- Copies of indoor case papers will be emailed in PDF format to the patient/authorised person within 3 days from the date of submission of approved application to the Medical Record department on chargeable basis
- Visiting hours from 4.00pm to 6.00pm.
- Children below 12 years are not permitted to visit patients.
- Patients can be admitted round the clock provided beds are available. All admission patients should undergo Registration process.
- Hospital Pharmacy services are available round the clock.
- Cafeteria Services Are Available For Patients, Relatives And Attendants From 7:00 A.M. To 9:00 P.M.
- Outside Food Is Not Allowed.
